Thief by Datamost 1981 (written by Bob Flanagan) is an Apple II Berzerk clone.

All game files copied from a highly protected / modified DOS 3.2:
  The protection was 10 sectors-per-track, synchronized tracks 4 and 5,
  two-byte and non-standard address and data prologues, non-standard
  epilogues.

Also includes a single file BRUNable version complete with title screen.
